US Club Card Request (Manager)

CONTEXT:

Requesting US Club cards has altered again as they are now being managed via GotSport. Many leagues and tournaments will require your team to use US Club-regulated registrations. This is a completely separate process than our usual North Texas Soccer Home Association Official USYSA Rosters.

STEPS:

• Complete Renegades Manager Request Form
• Ensure all players needing cards are on your GotSport roster. The US Club roster will be pulled directly from this roster.
  • • Players must have a legible birth certificate attached to their GotSport player profile.
    • Headshot Photo (no sunglasses, hats, or action shot photos – passport photo requirement preferred)
    • FORM #R002 | PLAYER INFORMATION, MEDICAL TREATMENT AUTHORIZATION, LIABILITY WAIVER/RELEASE AND CONSENT FORM - Download here
• Make Payment.
  • For Select teams (u11 and above): If these are NOT being used for your primary competitive league, the team is responsible for the cost of these registrations.
  • All Academy aged teams are responsible for the cost of each card. 
    • Staff (Competitive and/or Academy – Team Manager, Coach): $10 for registration (Also expect a $29 fee when completing background screening)
      
    • Player (Competitive- u11 and above): $25.25
    • Player (Academy - under u11): $19.25
    • The club will not apply for US Club card without payment. You can submit payment via:
